How do I fill out the NDMS application?

Below are the sections and fields that need to be filled out for the NDMS application.

Business Information section

  • Legal Business Name - include your PO Box if applicable.  Complete the Legal Address, City, State, and Zip.
  • DBA fields - include a physical address (this can be the field address or the president of the league’s house.
  • Select the appropriate business type (ie. Non-Profit, Government, etc).
  • In Business Since – rough month and year the league was started (doesn’t have to be exact but just a rough estimate).
  • Descriptor – name of the league or organization – this will showup on your customer’s credit card statements.
  • Federal Tax Identification Number - enter your EIN.
  • Incorporation State - two letter state abbreviation.
  • Publicly traded? - N/A.
  • Customer Service Phone – a phone number where one of your customers can reach someone. This will be added to your Contact Us section of the website and is a legal requirement.
  • Website Address – website address (URL) of the league or organization.
  • Contact Name, Contact Email, Contact Phone – this would be your treasurer most likely since they will receive the statements from NDMS.
  • Fax - leave this number as is.
  • Mail statements to my – select Legal.
  • By checking this box.... - leave checked.

Processing Summary section

  • This has been filled out for you and no changes are needed.

Bank Disclosure section

  • The Member Bank (Acquirer) Information on the left is the NDMS banking information.
  • For the Merchant Information on the right, type in your Merchant's Business Name.
  • Once the application is filled out, you will save the document and then print it out.  Here Principal 1 (see next section) will sign, date, and fill out where it says Merchant's Signature, Date, and Merchant's Name and Title.

Owners and Officers section

  • Fill out the form for just one Principal.
  • Principal 1 Name – the name of the person signing the application for the organization.
  • Position/Title – the organization’s title for the person who is signing the application.
  • Social Security # - leave blank for 501C3 (non-profit) organizations.
  • % Ownership – 0% for 501C3 (non-profit) organizations.
  • Home Address - leave blank for 501C3 (non-profit) organizations.
  • Drivers License # - the driver’s license # for the person signing the application for the organization.
  • State - the state where the driver’s license was issued for the person signing the application for the organization.
  • City, State - leave blank for 501C3 (non-profit) organizations.
  • Zip - leave blank for 501C3 (non-profit) organizations.
  • Home Phone – the phone number for the person signing the application for the organization.
  • Date of Birth – the date of birth for the person signing the application for the organization.

Security section

  • This has been filled out for you and no changes are needed.

Credit & Debit Card Rates and Fees section

  • This has been filled out for you and no changes are needed.

Banking Information section

  • Deposits/Withdrawals - this has been filled out for you and no changes are needed.
  • Name of Financial Institution - name of the organization's bank.
  • Bank Account #/DDA - organization's checking account #.
  • Routing Account #/ABA - organization's checking account routing #.
  • Phone Number/Contact Name - organization's local bank contact and phone number if available.

Merchant Signatures section

  • Once the application is filled out, you will save the document and then print it out.  
  • Here Principal 1 will sign and date where it says Signature, Principal or Corporate Officer #1.
  • You do not have to sign the Personal Guarantee.
